Working with NHS Sheffield, Space to Breathe are working with people in the Crystal Peaks. Hackenthorpe, Mosborough, Owlthorpe, Sothall, Gleedless, Beighton and Woodhouse areas of Sheffield to find out more about local working people, their wellbeing and their opinions and perspectives on health.

Using a piece of art (below) created by local Sheffield artist Lucy Freeman, we’d like you to consider the sense of connection you have to others, to your place of work and to the structure and support services available in Sheffield.

How connected do you feel in your community? What are the good points of connection? Are there any things that could be done to improve this sense of connection?

Once you’ve considered the image for a while we’d like you to complete a survey (which takes 15-20 mins) about your thoughts about connection, health and wellbeing.

After that survey, there is a short follow-up equality monitoring questionnaire about you but this is optional. This helps us make sure we are hearing from all of our communities. No personal information will be released when reporting and all information will be protected and stored securely in line with data protection rules.
